PCP or primary care physician practices medicine in family medicine, internal medicine and pediatrics, who provides initial care, acute, chronic and preventative care and is considered the gatekeeper for patient care.

Primary care physician takes care of all acute, chronic, preventative cares and other patient needs.
Provide age-appropriate vaccinations for children up to the age of 18, vaccines for adults like shingles, pneumococcal vaccine, flu vaccines, etc.
Screen for cancers in male & female patients for example breast cancer, cervix cancer etc.

You can request your prescription refills via logging on to your portal. You can call our office staff at 607-776-3522, 607-776-2932 to requested prescription refills.

My office offers same-day appointments, we accept walk-ins and appointments are also available via telehealth.

This is a secured HIPAA compliant communication between the office and the patient. Patients can request prescription, change appointment, request new appointments, and send us a message for their ongoing medical needs

Once you are an established patient with Kundlas family practice you will be given printed paper with activation code. You will go on the website & sign up with activation code, and set up your personal password.
My office will be available to help to set it up if you need additional help.

During the office hours Monday to Friday 9 AM to 6 PM, Saturday 9 AM to 12 noon office can be reached at 607-776-3522, 607-776-2932. Office staff will answer all the questions and send a message to the physicia. Patients messages will be answered within 24 hours. Urgent messages take priority.

We call all patients a day before their scheduled appointments.
You can also check your appointments on the portal and make changes if needed.
You appointments reminders will be also done electronically.

No-show means the patient canceled the appointment within 24 hours without notice. No-show is an inconvenience to those patients who need timely medical care
No-show will be recorded in the patient medical chart. After 3 no-shows office might consider dismissal of patient's care with proper written mail notice.

We have the office coverage 24/7. Until 5:00pm, patient can contact the office, after 5:00pm you can call the office number, the call will be transferred to answering service and they will connect you to the physican on call to answer your questions and advise you appropriately.
If you need your office medical records in the after-hours, you can log onto your portal and get your medication list, your problem list. If you need the medical record transferred, office will require authorized medical release and medical records will be sent electronically next working day.

You can also call the office at, offices 607-776-3522. The staff might be able to answer some of the questions, but the best practice is to call the billing company because they have all appropriate answers for you.

The co-pay and deductibles are expected to be paid the time of visit
We accept all major credit cards, checks and cash method of payment. If you we will be using health insurance to settle your account you must present your current insurance card at the time of each visit. This is a requirement of your insurance company. It also enables us to have most current information about the billing and your insurance company. Insurance company requires us to collect copayments at the time of service.
We have agreement with several insurance company who require us to bill them for the services provided to you, and accept as payment that amount specified in the agreement. You will be responsible for the co-pays, coinsurance, deductibles or services not covered by insurance.
We will file initial claim based on the information that you provided to the office. Under State law insurance company has 30-day in which to process and pay the claim, request more information, or deny the claim and notify us of the decision. If they have not notified us within 90 days of the date of service it will be assumed that insurance coverage is no longer in force and unpaid balance will be the patients responsibility
Past due accounts will be sent to collection agency.
Return checks
Checks returned due to insufficient funds or closed account will be charged $25. Patient is expected to pay with cash or credit card to cover the return check plus fee.
