Kundlas Family Practice files all insurance claims on behalf of the patient for both primary and secondary insurance. Below is a list of currently contracted and accepted insurance plans. If you have insurance that is contracted with Kundlas Family Practice, you are responsible for all applicable copays, coinsurance, deductibles and cost for any non- covered services you received on the day of service. Please understand that we cannot waive patient balances.
Your insurer maybe making changes to your coverage. Please contact them to verify our participation with your plan.
If you do not have insurance or are out of network, you will be responsible for payment of all the charges on the day of service.

Financial & Billing Policies
Our physician participates in most insurance plans and we will submit claims to these companies on your behalf.
Since every plan is different, please be sure to check your coverage if you have questions as to what your insurance will cover or pay for.To enable us to process your bill accurately, we will ask you for your insurance card(s) and check your picture identification each time you visit. This is to ensure that the information we have is correct, and that your plan is current and one in which we participate.
We are obligated by your insurance company to collect co-payments at each visit. This is an insurance company policy. We accept cash, checks, debit cards and all major credit cards. Please make sure that you have your co-pay at the time of your visit.
We will process your medical claims directly to your insurance carrier for you. While we make every effort to resolve insurance issues, please remember that you are ultimately responsible for your insurance benefits, as well as the cost of your health care. You should address and questions or complaints regarding your coverage to your insurance carrier or their agent.

A deductible is an amount that you are required to pay out of your own pocket up to a certain limit before a higher level of insurance benefit is available to you. Although not everyone has a deductible policy, they are becoming more common. A deductible can be as small as $50 or as large as $5,000 or more.
